WHAT ARE OFFICE HOURS?

The Center for Athletic Medicine, Ltd office hours are Monday through Friday 8:00 am to 4:30 pm.

CAN YOU GIVE ME DIRECTIONS?

The Center for Athletic Medicine, Ltd is conveniently located at Diversey Parkway and Halsted Street.  For those patients who use public transportation, we are just east of the Diversey Brown/Purple Line CTA stop. Located minutes from downtown 

Chicago and from the I 90/94 expressway off of the California exit. From Lake Shore Drive, we are off of the Fullerton exit.  MAP >

HOW DO I MAKE AN APPOINTMENT?

All office visits are by appointment. To schedule an appointment, please call our office at (773) 248-4150. We will make every attempt to schedule an appointment that is convenient for you. WHAT INSURANCE DO YOU ACCEPT/WHICH NETWORKS DO YOU PARTICIPATE IN?

The Center for Athletic Medicine, Ltd accepts most major insurance plans. We recommend that you call your insurance carrier to verify if our doctors are in your plans network; prior to your appointment.

We ask that you present your insurance card at each appointment so that we have your most current insurance information. Please let us know if you have any changed your address and/or telephone number. We also ask that co-payments be paid at the time of service.

WHAT ABOUT X-RAYS? WILL MY INSURANCE COVER X-RAYS?

We do have x-ray equipment located within our facility for your convenience. If you have an HMO, we make arrangements to have X-rays done that your insurance will cover, whether that is in our office or at another facility. 

DO I NEED A MEDICAL REFERRAL FROM MY PRIMARY HEALTHCARE PROVIDER?

The Center for Athletic Medicine, Ltd is considered a specialty practice, therefore HMO insurance plans require a referral from your primary care physician before you may be seen by one of our physicians.

To avoid billing issues, we ask that you contact your primary care physician office and inquire whether or not a referral is needed before scheduling an appointment with us. Please be sure to bring your referral with you to your visit.

HOW DO YOU HANDLE WORKER’S COMPENSATION CASES?

Many of our patients are referred to us through their employers or insurance companies. We ask that your bring all medical records, x-ray films and insurance documents (including WC case number) with you to your appointment. Our office will assist in making sure everything is in order by the time of your appointment.

WHAT IF I NEED A PRESCRIPTIONS REFILL?

Patient contacts their pharmacy and has the pharmacy send a refill request and then our office takes care of this. For pain medication, please contact our office.

WHAT FORMS DO I NEED TO FILL-OUT? ARE THEY AVAILABLE ONLINE?
